Background Aldo-keto Reductase 1C4 (AKR1C4) is a member of the aldo-keto reductase (AKR) superfamily. AKR1C4 catalyzes the reversible oxidoreduction of various 3α-hydroxysteroids and prostaglandins as well as the oxidation of trans‑dihydrodiols of aromatic hydrocarbons and alicyclic alcohols. AKR1C4 acts as NADPH-dependent 3-, 17- and 20-ketosteroid reductases and as 3α-, 17β- and 20α-hydroxysteroid oxidases. AKR1C4 reduces 11-ketodihydrotestosterone (11KDHT) to both 3α- and 3β-hydroxy metabolites. AKR1C4 is important for steroid hormone metabolism primarily through synthesis and clearance of testosterone.
